Please note that rehearsals, etc. are additional and need to be scheduled in advance. Your balance will include a travel fee for any weddings outside of 25 miles, church certificate fees, etc., so please be sure of the exact amount due before using this form. Deposit needs to be at least $250 and is non-refundable.
Due to the higher fees for the coming year implemented by the credit card processing companies, there will be a $15 fee assessed to any payment made by credit card or PayPal. The preferred manner of payment is Venmo. Thank you for your understanding.
